Quality Partners of Rhode Island (Quality Partners) is pleased to announce the promotion of Lynne Chase as Program Administrator for our Safe Transitions team. In her new role, Lynne will provide day-to-day leadership for our Safe Transitions project.

As the Quality Improvement Organization for the State of Rhode Island, Quality Partners of Rhode Island works on the 9th Statement of Work with the Centers for Medicare & Medicaid Services (CMS). The Safe Transitions team focuses on improving coordination across the continuum of care, which is part of CMS's National Patient Safety Initiative (NPSI).

Quality Partners is proud to be one of 14 states awarded a competitive Medicare contract to expand the NPSI. The Safe Transitions Project furthers Rhode Island's leadership around care coordination, already evidenced by the Continuity of Care Form. The project:

* focuses on patients’ discharges from the hospital to other care settings,

* promotes cross-setting communication, and

* ultimately, aims to improve patients' transition experiences, self-management skills and outcomes.

Lynne has been working for Quality Partners of Rhode Island since 2008 as a Program Coordinator for the CMS Medications Measures Special Study, as well as in a supporting role with the Chronic Kidney Disease team. In that role, she maintained project work plans, managed day-to-day project operations, including project communications and meeting facilitation and established positive business relationships with critical partners.

Prior to her work with Quality Partners, Lynne served as project manager for CVS in Woonsocket.
